Completed the survey twice, but still says incomplete. Anyone else having this issue?

If you are doing it from a browser, click on the survey link directly--don't try to open it in a new tab. I generally open things in new tabs, and it never worked for me until I opened it in the same tab. Hopefully, that's your problem
